Please be advised that this warranty form is strictly for parts that have been damaged or require replacement under the terms of our warranty policy.

To ensure correct processing:

**– The original (old) part must be returned to Lochinvar within 30 days of the warranty request submission.**

**– Failure to return the part within this 30-day window will result in cancellation of the warranty claim. A replacement part will not be shipped if the original part is not received.**
